適用於≥1/3視盤直徑的大型裂孔或初次手術未閉合的病例。將剝離至裂孔邊緣的ILM翻轉覆蓋於裂孔上,或填入裂孔內。再次手術時,可將從黃斑外剝離獲得的游離ILM填入裂孔以促進閉合。一項對4項小型RCT的系統性回顧表明,該技術可能比標準ILM剝離帶來更好的BCVA改善(低證據),閉合率更優(中等證據)3)。
Szeto等人(2025)報告了針對大型裂孔(包括>30%伴有高度近視,MLD≥500 μm)的無氣體手術16)。無氣體組閉合率為94.1%,傳統組為95.2%(P=0.812),兩者相當。術後早期視力(術後1週和1個月)無氣體組顯著更優。中心凹膠質增生率無氣體組較低(4.9% vs. 20.0%,P=0.043)。術後次日即可進行OCT評估,且患者無需承受體位限制的負擔16)。
Yu等人(2025)的統合分析(8項研究)顯示,LHEP保留手術組的術後BCVA改善量為−0.25 logMAR(95% CI −0.30至−0.21,P<0.00001),具有顯著性11)。與傳統剝離術相比,BCVA改善量的差異為−0.19 logMAR(P<0.0001),保留組較優。術後EZ修復的患者比例在保留組也顯著較高(OR 2.55;95% CI 1.48至4.38)11)。LHEP保留組未報告術後FTMH形成11)。
