Varshney et al.(2021)は、CXL後角膜炎にACHを合併した症例を報告した7)。CXL施行済みの角膜では21日で急速に消退し、通常の2〜4か月と比較して著明に短縮された。CXLによるコラーゲン架橋結合の強化→前方実質のinterlamellar cohesive strengthの増大→膨潤抵抗性の増大が機序として示唆された。架橋済み角膜は酵素的消化に対する抵抗性が高く、これが早期消退に寄与すると考えられている。
Kaur et al.(2025)は7例(年齢中央値16歳、術前角膜厚1120〜2363μm)に対してiOCT誘導下の部分層圧縮縫合(PTS、目標深度50〜60%)を施行した2)。iOCTにより「caterpillar」様外観を呈するstromal compactionを確認しながら縫合を行い、全例が2週間以内に消退、縫合は8〜12週後に抜糸された。PTSは全層縫合(FTS)と同等の有効性を示しながら内皮損傷リスクが低いことが示唆された。
