眼内再発時には角膜後面沈着物(KP)の頻度が初発時より増加し(47.4% vs 29.4%)、星状またはmutton fat様のKPが出現する2)。星状KPは角膜後面に形成される沈着物で、PVRLに比較的特異的な所見として報告されており、ぶどう膜炎との鑑別に参考となる(単独では確定診断はできない)2)。再発時には網膜内沈着物が47%で認められ(初発時7%より有意に多い)、再発の64.3%は片眼性で生じる2)。

追跡期間中の眼内再発率は約27.5%(平均42.5か月追跡で51例中14例)であり、再発時の最も多い所見は硝子体混濁(84%)である2)。文献全体では眼内再発率は2.5〜59.6%と施設・報告によって幅がある2)。全身化学療法と硝子体内化学療法の併用が眼内再発リスクを低減させる可能性がある。硝子体内化学療法の既往がないことが眼内再発の独立したリスク因子として報告されており(OR 7.72; 95% CI 1.37-43.6)2)、積極的な眼局所治療の重要性が示されている。定期的なIL-10測定が再発の早期発見に推奨される2)。
